02 Pregnancy history

Tell us about your previous pregnancies and births.

Include all pregnancies, including pregnancy losses and a current pregnancy.
Count a twin or multiple birth as one delivery.
Deliveries at 37 weeks or later.
Any complications or other details you would like to share. Please leave out children’s names.
03 Health & practical details

Optional — you can also add these details later in your account.

Relevant conditions, operations or medicines you would like the team to know about.
For example, support with appointments or childcare. No names are needed.

GENOVIA LLC will keep your registration for up to 12 months from your last profile update or sign-in. Read the Candidate Privacy Notice before continuing.
